Repair Authorization and Estimate Approval

1. What I am authorising. Diagnosis and the work described here: work description, for the estimated amount of estimate amount, including parts and labour but not tax unless stated. I authorise business name's staff to operate the vehicle on roads and on the premises for testing and for moving it.
2. The estimate is an estimate. If the work turns out to need more than the estimate, business name will stop and contact me on customer phone before going further, and will not exceed estimate amount by more than overage allowance without my agreement recorded in writing. A supplemental authorization covers each additional amount.
3. Diagnosis charge. If I decline the work after diagnosis, the diagnostic charge of diagnostic fee is still due, and the vehicle is reassembled to the state described here: reassembly terms.
4. Parts. The parts used are parts type — new, remanufactured, aftermarket or used as stated. Old parts are returned to me if I ask before the work starts, except parts that must be sent back under a warranty or exchange, or that the law requires to be kept.
5. Warranty. The work carries warranty terms. A warranty claim is made to business name first, and work done elsewhere in the meantime is not covered.
6. Personal property. I have removed anything of value from the vehicle. business name is not responsible for personal property left in it, or for damage from a pre-existing condition, a fault not related to this work, or road hazards during testing.
7. Collection and storage. I will collect the vehicle within collection days days of being told it is ready. After that, storage is charged at storage rate per day, and business name's rights over an uncollected vehicle are as the law allows.
By signing I authorise the work described, and I confirm that I have read this and meant to sign it.
