What this document is

A short document, signed by both sides at handover, recording the condition of a thing and what the person taking it is responsible for.

Not a feature list. These are the three moments this exists for.

Two recollections and no record

Yours and theirs, both sincere, both different. Without something made at the time and agreed by both, this ends in whoever cares less about the relationship winning.

Photographs that live in a camera roll

Taken at handover by whoever was working, on their own phone, and findable only by scrolling. They prove nothing about when, and they leave with the member of staff.

The charge that gets dropped to keep the peace

Not because it was wrong, but because defending it costs more than the damage. That is a real cost and it is invisible, which is why nobody ever fixes it.

Both people looking at the same thing, the notes typed once and agreed, and a signature from the person taking it.

Part of the sealed record and part of the evidence bundle, with the time they were taken, rather than a folder somebody has to find.

A second acknowledgment closes the loop, so the file holds the state at handover and the state at return with the same signatures behind both.

We are not making a judgement about the damage

This records what two people agreed the condition was, at a time, with a signature. It does not assess, price or adjudicate anything, and whether a charge is fair or enforceable where you operate is a question of your terms and your state's consumer rules, which the starter marks for an attorney.

Can this be part of the rental agreement rather than separate?

Either. Some businesses want one document; most want the agreement signed in advance and the condition recorded at the counter, which is two documents behind the same booking.

How many photographs can be attached?

Enough for a walkaround. They are stored with the document, included in the evidence bundle, and deleted on the retention schedule you set rather than kept forever by default.

Does the customer get a copy?

Yes, the same way they get any signed document — a copy and a certificate, which is also the reason this works: they have the record too, so it is not your word against theirs.
