Membership Liability Waiver

Between business name and guest name, on date.

1. What I am doing. I am using business name's facilities, equipment and classes. I understand that exercise carries risks including strain, injury from equipment, collision with others, and in rare cases serious injury or death, and I accept those risks for myself.

2. Fitness and health. I am fit to exercise, or I have been told by a doctor what I should not do and will follow that. I will tell staff about any condition that could affect my safety before I take part, and I will stop and tell staff if I feel unwell.

3. Instruction and equipment. I will follow the instruction given, use equipment as shown, and ask before using anything I have not been shown.

4. Release. To the extent the law where business name operates allows, I release business name, its owners, staff and instructors from claims for injury or loss arising from my ordinary use of the facilities, other than claims arising from their negligence or misconduct that the law does not let a release cover.

5. Belongings. business name is not responsible for belongings left in lockers or on the floor.

6. Medical care. If I am injured, I consent to first aid and to emergency medical care, and I am responsible for its cost.
