Guardian Consent and Activity Permission

I, guardian name, am the parent or legal guardian of participant name, born date of birth, and I sign this on date for program name at business name, running from start date to end date.

1. Permission. I give permission for the child to take part in program name and in the activities it includes, which are: activities. I have read what each involves and I am satisfied the child can take part.

2. Off site. I give permission for the child to leave the site for the trips listed here — trips — supervised by business name's staff, and to travel to them as set out in the transportation authorization.

3. Supervision and behaviour. I understand the ratio of staff to children is supervision ratio, that the child is supervised during programme hours and not before or after them, and that I am responsible for the child at all other times. I have read the code of conduct and I understand that a child who repeatedly breaks it may be sent home at my cost.

4. Collection. The child may be collected only by me or by the people named here: authorised collectors. Anybody collecting the child may be asked for identification.

5. Health. The child's health, allergies, medication, dietary needs and anything staff should watch for are written here: health notes. Medication is handed to staff labelled and in its original container, with written instructions.

6. Sun, water and heat. If the programme includes swimming or open water, the child's ability is: swim ability. I understand that a child whose ability is not what I have written here is at risk, and I have written it accurately.

7. Photographs. Whether the child's image may be used, and where, is set here: photo permission. If I say no, that answer is respected in every publication and posting.

8. The risks. Group activity with children carries risks that supervision reduces and cannot remove. I accept the ordinary risks of the activities listed, and nothing here waives a claim the law does not allow a guardian to waive on a child's behalf.

By signing I confirm that I am the child's parent or legal guardian, that I have read this, and that I meant to sign it.
