Guardian Consent for a Minor

I, guardian name, am the parent or legal guardian of guest name, born date of birth, and I sign this on date for their participation in activity with business name.

1. Consent. I consent to the minor taking part, and I have read the participant waiver they are asked to sign. I agree to its terms on the minor's behalf to the extent the law where business name operates allows a guardian to do so.

2. Supervision. I understand that the minor is supervised by business name's staff during the activity and not before or after it, and I am responsible for the minor at all other times.

3. Health. The minor has no condition that would make participation unsafe, other than any I have written here: health notes.

4. Medical care. If the minor is injured and I cannot be reached, I consent to first aid and to emergency medical care, and I am responsible for its cost.

5. Contact. I can be reached on guardian phone during the activity.

By signing I confirm that I am the minor's parent or legal guardian, that I have read this, and that I meant to sign it.
