Several states require a specific document, with specific wording, acknowledged separately — a general liability waiver does not satisfy it and folding the two together can invalidate both. Either guardian can sign, and the record says which one did. If you need both, list both as signers and the document is not complete until both have.
They do if you set an expiry, and for an annual membership you probably should. Reminders go out before the date rather than after.
