Guardian Consent for a Minor Participant
I, guardian name, am the parent or legal guardian of guest name, born date of birth, and I sign this on date for their participation in program at business name.
1. Consent. I consent to the minor taking part, and I have read the waiver participants are asked to sign. I agree to its terms on the minor's behalf to the extent the law where business name operates allows.
2. Concussion and injury. I understand that contact and impact can cause concussion, that the signs may appear later, and that a participant suspected of concussion will be removed from the session and not returned without a doctor's clearance.
3. Health. The minor has no condition that would make participation unsafe, other than any I have written here: health notes.
4. Medical care. If the minor is injured and I cannot be reached, I consent to first aid and to emergency medical care, and I am responsible for its cost.
5. Pick-up. The minor will be collected by me or by the people named here: authorized pickup. I can be reached on guardian phone.
By signing I confirm that I am the minor's parent or legal guardian, that I have read this, and that I meant to sign it.
