Youth Trip Guardian Consent
I, guardian name, am the parent or legal guardian of participant name, born date of birth, and I sign this on date for the trip trip name with business name, leaving departure datetime from departure point and returning return datetime.
1. Permission. I give permission for the young person to go on the trip, to stay at accommodation, and to take part in the activities planned: activities.
2. Who is leading. The trip is led by trip leader, reachable on leader phone, with adult count adults for youth count young people. The adults have been screened as business name's safeguarding policy requires.
3. Travel. I authorise travel by transport, including by a carrier business name contracts. I understand that the schedule may change for weather, traffic or safety.
4. Conduct. I have read the trip's code of conduct with the young person. I understand that a young person who breaks it seriously may be sent home at my cost, and that I will be told first.
5. Health and medication. Allergies, conditions, medication and dietary needs are written here: health notes. Medication is handed to the leader labelled and in its original container.
6. Emergency medical care. If the young person is injured or becomes ill and I cannot be reached in time, I authorise the leader to obtain emergency medical care, including transport by ambulance, and I am responsible for its cost. I can be reached on guardian phone, and if not, second contact on second phone.
7. Money and property. The young person is responsible for their own money, phone and property, and business name is not.
8. Photographs. Whether the young person's image may be used, and where, is set here: photo permission.
9. Risks. Travel and group activity carry risks that supervision reduces and cannot remove. I accept the ordinary risks of what is planned, and nothing here waives a claim the law does not allow a guardian to waive on a young person's behalf.
By signing I confirm that I am the young person's parent or legal guardian, that I have read this, and that I meant to sign it.
