Guardian Consent for a Minor Passenger
I, guardian name, am the parent or legal guardian of guest name, born date of birth, and I sign this on date for their place aboard vessel name with business name.
1. Consent. I consent to the minor going aboard and taking part in the trip, and I have read the passenger waiver. I agree to its terms on the minor's behalf to the extent the law where business name operates allows a guardian to do so.
2. Aboard and ashore. I understand that the minor is in the crew's care from boarding until the vessel is tied up and they step ashore, and not before or after that, and that I am responsible for the minor at all other times. An adult responsible for the minor aboard is responsible adult.
3. Life jackets. I understand that the minor will be required to wear a life jacket at the times the captain and the law direct, and that a minor who will not wear one may be kept ashore.
4. Health. The minor has no condition that would make the trip unsafe, other than any I have written here: health notes. This includes anything affected by motion, sun or heat.
5. Medical care. If the minor is injured and I cannot be reached, I consent to first aid and to emergency medical care, and I am responsible for its cost.
6. Contact. I can be reached on guardian phone while the vessel is out.
By signing I confirm that I am the minor's parent or legal guardian, that I have read this, and that I meant to sign it.
